The Cubby Insurance Support Program acts as your personal guide through the insurance process, coordinating with your doctor, reviewing paperwork, and connecting you with a medical supplier for your claim submission. You can work with any medical supplier of your choosing, however, this program is available through select suppliers only.
Step 1: schedule doctor's appointment
Set up an appointment to gather the documents insurance requires:
- Prescription (Rx): Set up an appointment with your loved one's doctor to discuss the sleep and safety challenges you are having. Then, request a prescription (Rx) to be completed.
- Letter of medical necessity (LMN): Discuss the LMN with your Occupational Therapist (OT) or Physical Therapist (PT). If you do not have an OT or PT, your doctor can write the LMN.
Note: If you have Minnesota Medicaid, your LMN must be completed by a Behavioral Therapist (BCBA) or OT. If not using Medicaid, a doctor, PT or OT can complete it.
If you have Colorado Medicaid, your loved one's doctor is required to sign this Questionnaire #19 form, along with a face-to-face office visit note documenting your discussion of the Cubby Bed.
Step 2: upload insurance cards
Use the secure portal link from the email or text message to upload the front and back of your loved one's insurance cards. Please upload both your primary and secondary insurance cards if you have secondary insurance.
Step 3: check your email for guidance
You’ll get an email from our team outlining your next actions. When it’s time to share your documents, we’ll provide a secure link to upload your insurance card and any medical documents. If you have any questions, feel free to reach out to Care@CubbyMedical.com.
