Supplier Locator + FAQs

We partner with hundreds of third-party Medical Suppliers around the US. They'll help you gather documents and submit to insurance, but you must obtain your prescription and Letter of Medical Necessity for a Cubby Bed first.

FAQs

To move forward with insurance coverage for your Cubby Bed, you'll need a prescription for a Cubby Safety Bed from your doctor, and a Letter of Medical Necessity (LMN), which can be provided by your doctor or an Occupational Therapist (OT) or Physical Therapist (PT) if you have one.

Make sure to complete the following steps:

  1. Schedule an appointment with your doctor.
  2. Download the Required Documents and provide it to your doctor, who will complete the necessary forms.
  3. Request the prescription and Letter of Medical Necessity.

Once you have both documents, you can connect with a Medical Supplier to proceed with the insurance process!
